We have dental patient forms to fill out, giving us an opportunity to know you. This way, when you come to see us, we are ready to make your visit as pleasant and smooth as possible.
New Patient Forms
If you are a new patient and have never been to San Marcos Dental Studio, we kindly ask you to complete our new patient forms, accessible at the bottom of this page.
The forms are three pages / 3 sections, which include Patient Information, Insurance Information (if applicable), Medical History, and Dental History.
We recommend being as detailed and complete as possible to provide San Marcos Dental Studio with the information we need to assist you appropriately.
You may fax them completed to 760-471-9574 or bring them with you at the time of your appointment to reduce wait times.
